With AndHealth, our mission is to help you feel better and live your fullest life with Psoriasis and/or Psoriatic Arthritis. We will help you get your autoimmune symptoms and inflammation under control.
Your care team will work with you to easily + quickly book appointments, give you dedicated pharmacy + insurance support for your biologics, and co-create a care plan to improve your plaques and patches, itching, flaking or pain, as well as your sleep, stress, nutrition, and root cause triggers of flares + symptoms. Your first 60 min dermatology appointment is usually available within 1-2 weeks.

A dermatology team dedicated to you
A 5-person care team will surround you with continuous care and give you a personalized treatment plan that covers both medications and tools for your everyday life. The full care plan aims to help you reduce inflammation, manage symptoms and improve your overall health.

Dermatologist
- Evaluation of plaques and patches/rashes, and any + all other symptoms (even if they do not seem related)
- Confirm your diagnosis(es)
- Medication management
- Supplement recommendations
- Creates your care plan and monitors your care
Clinical Pharmacist
- Helps ensure medications, including/especially your biologics, have the needed Prior Authorizations, Copay and Patient Assistance programs on file to give you the lowest cost, and no interruption in refills
- Explains how medications work individually and together
- Explains how supplements help your symptoms
- Answers questions about how to take your meds, why your meds were prescribed, and any other q's about your medications and supplements
- Medications can be delivered to your door, or picked up in person. In-person pick-up can include injection training as needed
Coordinating Provider (NP, MD)
- Communicates care plan developed with all input from your rheumatologist, pharmacist, coach and patient navigator
- Coordination with your primary care and/or mental health providers
- Sleep strategies and tracking
- Movement therapy
- Nutrition and meal planning
- Stress management
Health Coach
- Your cheerleader, supporter and go-to for all elements of your care plan
- Helps create a new routine for taking your meds, if needed
- Develops goals for moving more or eating fewer foods that trigger pain and flares
- Helps identify root causes for symptoms and flares, such as diet habits or sleep issues
- Works with you however often is helpful
- Communicates over text, phone or zoom and checks in on you frequently
Patient Navigator
- Your go-to for appointment scheduling and changes anytime needed
- Helps with navigating your insurance plan, your insurance benefits, etc
- Helps find programs that you qualify for to help with transportation, housing, food assistance etc.
- Your dedicated person to reach out to for questions and help from your AndHealth team

Medicaid Plan
No copay for any provider visit (dermatologist, coordinating provider, health coach, clinical pharmacist, patient navigator).
Orders for tools and tracking to manage your condition are also provided at no cost to you, e.g., lab work, imaging, nutrition programs and/or sleep tracking.
Medicare Plan
Depending on your Medicare or Medicare Advantage plan, your copay will vary. Typically, a specialist copay is charged for dermatology visits and a primary care copay for NP visits. Pharmacist, health coach and patient navigator provided with no copay, along with other tools to manage your autoimmune care.
Group or Individual Plan
Copays include a specialist copay for dermatology visits and primary care copay for nurse practitioner visits, depending on your health plan type (e.g., HMO, PPO, HDP). Pharmacist, health coach and patient navigator are provided with no copay, along with other tools to manage your autoimmune care.
Uninsured
One of our care navigators will work with you to find primary and dermatology care that you are qualified for at a Community Health Center. Additionally, they will research which assistance programs you may qualify for to help with housing, transportation, groceries and utilities.
