Rheumatologist

  • Evaluation of pain, swelling, and other symptoms
     
  • Confirm your diagnosis(es)
     
  • Medication management
     
  • Supplement recommendations

Clinical Pharmacist

  • Helps ensure medications, including/especially your biologics, have the needed Prior Authorizations, Copay and Patient Assistance programs on file to give you the lowest cost, and no interruption in refills
  • Explains how medications work individually and together
  • Explains how supplements help your symptoms
  • Answers questions about how to take your meds, why your meds were prescribed, and any other q's about your medications and supplements
  • Medications can be delivered to your door, or picked up in person. In-person pick-up can include injection training as needed

Coordinating Provider (NP, MD)

  • Communicates care plan developed with all input from your rheumatologist, pharmacist, coach and patient navigator
  • Sleep strategies and tracking
  • Movement therapy
  • Nutrition and meal planning
  • Stress management
  • Coordination with your primary care and/or mental health providers

Health Coach

  • Your cheerleader, supporter and go-to for all elements of your care plan
  • Helps create a new habit or routine for when to take your meds
  • Develops goals for moving more or eating fewer foods that trigger pain and flares
  • Helps identify root causes for symptoms and flares, such as diet habits or sleep issues 
  • Works with you however often is helpful
  • Communicates over text, phone or zoom and checks in on you frequently

Patient Navigator

  • Your go-to for appointment scheduling and changes
  • Helps with navigating your insurance plan, your insurance benefits, etc
  • Helps find programs that you qualify for to help with transportation, housing, food assistance etc.
  • Your 24x7 person to reach out to for questions and help from your AndHealth team

How to estimate the cost of your healthcare with AndHealth

We aim to provide the highest quality care at the lowest cost for your plan benefits. We are here for you!

Provider visits: No copay for appointments for rheumatologist, coordinating provider, health coach, clinical pharmacist, patient navigator
 
Medication + orders for your condition: Provided at no cost, e.g., prescriptions, lab work, imaging (x-ray, MRI)
 
Treatments to improve your symptoms: Provided at no cost, e.g., nutrition programs, sleep tracking
Provider visits: Copay varies by type of Medicare or MA plan. Typically, a specialist copay is charged for rheumatology visits, and a primary care/office copay for NP/MD visits. Clinical pharmacist, health coach and patient navigator are provided with no copay.
 
Medication: Costs according to your health plan. If qualified after secondary insurance and drug coupons are applied, a financial assistance program is available to help with medication deductible as part of Medicare Part D plan.
 
Orders to track your condition: Costs according to your health plan, e.g., lab work, imaging 
 
Treatments to improve your symptoms: Provided at no cost, e.g., nutrition programs, sleep tracking
Provider visits: Copay varies by plan type (e.g., HMO, PPO, HDP). Typically, a specialist copay is charged for rheumatology visits, and a primary care/office copay for NP/MD visits. Clinical pharmacist, health coach and patient navigator are provided with no copay.

Provider visits: One of our Care Navigators will work with you to find primary and rheumatology care on a sliding fee scale, even if you do not have an insurance plan right now. Additionally, they will help you find other assistance programs you may qualify for to help with housing, transportation, groceries and utilities
 
Treatments to improve your symptoms: Provided at no cost, e.g., nutrition programs, and sleep tracking
